    A 72 years old male presents with 3 months history of general weakness and slight difficulty walking. He reports that his symptoms get better as the day progresses. On further questioning, he admits that he has had a non productive cough and he has lost some weight. Past medical history includes COPD and hypertension. He is currently taking amlodipine and salbutamol. He is an ex smoker but previously smoked 40 a day for 30 years. On examination, he appears cachetic and pale. There is scattered wheeze on auscultation of his chest. Power is 4/5 in all 4 limbs and he is hyporeflexic. His plantars are down going. There are no other neurological findings. What is the most likely underlying condition?
